UIL/RankOne Sports Forms

All Yearly Forms (Except Medical History and Physical Evaluation) can now be filled out on RankOne Sports Website at:

https://moodyisd.rankonesport.com/New/NewInstructionsPage.aspx

Student ID is required to properly fill out documentation
Student ID can be found in ParentPortal or by calling the Main Office

****The Only document that cannot be completed on this website is the Physical Form and Medical History****

If you are unable to access the website, the documents may be downloaded and printed from the links below.

Step 1: Once you are on the RankOne forms website, you will need to read through the information listed on the screen. After doing so, you will need to click the “Click Here” Tab to go to the online forms.

Step 2: Once on this page, a RankOne account will need to be created for all fist time users of the website.

Step 3: Once your account has been created and verified, you will return to this page and log into your newly or pre-existing account.

Step 4: Lastly, once you are signed in you will reach this page. On this page you will enter your students last name along with their Student ID number and complete the required docume
